The Healthy Lifestyles Program, a diabetes prevention program, focuses on behavior change, movement, weight loss and lifestyle modification. It follows a curriculum produced by the CDC (Center for Disease Control and Prevention). The goals of the program are for participants to avoid or delay Type 2 Diabetes, lose weight and increase physical activity to at least 150 minutes per week or more.

HLP meets twice a week for 12 weeks. One meeting is classroom based and the second meeting is in the fitness center. 

The curriculum is evidenced based and uses outcome research thus providing participants a success path forward to a more permanent healthier lifestyle.

Participants receive a 12-week family membership during the program.

Pathway Experience

In our efforts to provide a platform of togetherness and safety, and by the graciousness of our donors, the pathway experience will consist of five, essential modules:

  • Access to our virtual wellness platform, Y Wellness 24/7
  • Weekly connections and conversations via ZOOM with your pathway family.
  • Pre/Post survey and InBody scan
  • Weekly Togetherness Times will be established by the pathway family by week two. Togetherness Times has shown days/times during the week that the majority of pathway participants plan on accessing a YMCA of Bucks and Hunterdon Counties branch. YMCA of Bucks and Hunterdon Counties will have qualified staff standing by during Togetherness Times for support and connection.
